Community Partnerships

What are Community Partnerships?

Community Partnerships are networks of statutory, voluntary and community organisations that work together to address inequalities and improve outcomes for their local populations.

We have thirteen Community Partnerships within Bradford District and Craven, based on Primary Care Network (PCN) population footprints of between 30,000 and 50,000 residents. The overarching ethos is to collaborate with communities at a local and hyper-local level – we refer to this as ‘inverting the power to act’. We believe that by empowering the diverse communities we serve, by joining them up with the NHS, makes it easier to live healthier, happier lives.

The thirteen Community Partnerships each have local priorities based on their own population health needs and work closely with primary care, local authorities, local people and the Voluntary, Community and Social Enterprise (VCSE) sector to identify and address inequalities in health and care for their local populations.

Over 250 projects are currently being delivered in 2024/25 across six localities (detailed below) covering a diverse range of needs and  integrating a range of health and care services.
