Let’s tell you a bit about OUR VISION

The Healthy Hearts Programme ambition is to reduce cardiac events by 10 % across the place. To reduce health inequalities. To empower people to make healthy heart choices in their daily lives to prevent and reduce heart disease and associated illness including Stroke, cardiac arrest.

We hope to optimise the management of those with cardiac disease, to reduce risk of deterioration and ensure they have the best quality of life possible. By reducing the occurrence of heart disease and cardiac illness, we hope to level up and improve the life expectancy of our population and improve their quality of life.

This will be delivered through community engagement, improved access to care across all sectors, achieved by working with all stakeholders with the united goal to reduce Cardiovascular disease and prevention of this disease for future generations. If we achieve this goal it will help people stay Healthy, Happy and at Home.
