If you are a GP within Bradford District and Craven and have a patient presenting with Long COVID symptoms, for 4 weeks or more, which do not improve with self-management, please refer to the Long COVID Service.

We offer patients a personalised rehabilitation pathway, including virtual and face-to-face (where appropriate) group therapy sessions run by Long COVID specialists in order to support patients to self-manage, improve or resolve their symptoms.

Inclusion/exclusion criteria

Inclusion criteria

  • Individuals experiencing persistent symptoms believed to be related to a suspected or confirmed Covid-19 infection.
  • Aged 18+. Ages 16–18 may be considered on a case-by-case basis in consultation with the GP or Paediatrician for Long Covid – self-referrals not accepted for this age group.
  • If already under another NHS service, referral must include a clear rationale for how this service complements (rather than duplicates) existing support.
  • The patient must be ready and willing to engage with the service.

Exclusion criteria

  • Actively suicidal.
  • Documented substance, drug, or alcohol abuse.
  • Severe or unstable mental health conditions requiring secondary care (e.g. psychosis, unstable bipolar disorder).
  • Terminal prognosis of less than 12 months (e.g. DS1500 or SR1 confirmation).
  • Patients who are more appropriately supported by an already established NHS service.

How to refer

The red flag screening tests we advise would be based on patient symptoms (see guidance on GP assist) but usually include:

  • Chest x-ray
  • Routine and fatigue bloods
  • Electrocardiogram (ECG) – a test that can be used to check your heart’s rhythm and electrical activity

Once completed, please refer the patient if symptoms do not resolve with self-management and there are no other obvious causes for these symptoms after 4 weeks.
