Multi-system rehabilitation service: Supporting you to take control of your health
What is rehabilitation?
Rehabilitation is a personalised process that helps you recover, manage, and improve your health and daily functioning after illness or injury. It focuses on helping you regain control over your body and mind so you can live as fully and independently as possible.
Our approach recognises that real progress comes from looking at the whole person; their physical symptoms, mental wellbeing, and how these affect everyday life.
Supported self-management: Why taking control matters
This service is built on the principle of supported self-management. That means you are not a passive recipient of care; you are an active partner in your rehabilitation journey.
Supported self-management means we provide you with the tools, knowledge, and support you need to take control of your own health. You learn to:
- Understand your condition and symptoms
- Make informed decisions about your care
- Build skills to manage your symptoms day to day
- Set realistic goals and track your progress
While you will attend group sessions and appointments as part of the service, much of the work needed for your recovery will take place outside of these sessions, in your daily life. This might include exercises, pacing your activities, and applying strategies you learn in the sessions. Without this ongoing effort outside appointments, improvements may be much slower or may not happen at all.
Research shows that people who actively manage their own health tend to recover faster, feel more confident, and maintain improvements for longer. By working together as a team, you lead your recovery while we guide and support you every step of the way.
About the multi-system rehabilitation service
Our service is designed to support adults with ongoing health issues affecting multiple systems in the body, such as persistent Long Covid symptoms or other long-term conditions. We take a holistic, whole-person approach, considering how physical, mental, and social factors interact and impact your wellbeing.
This is a supported self-management service that combines professional guidance with patient empowerment. Through online group sessions, personalised rehabilitation plans, and a range of self-management resources, you are supported to take control of your health and achieve meaningful, lasting improvements.
You are a key member of the rehabilitation team; your active participation and commitment are essential to your success.
Pathways within the service
The multi-system rehabilitation service offers two pathways, depending on your individual needs: the long COVID pathway and the vocational rehabilitation pathway. Both are built on the principles of supported self-management and whole-person care.
Long COVID pathway
This pathway supports people who are experiencing ongoing symptoms that may be linked to a previous Covid-19 infection. These symptoms can affect different systems in the body, including breathing, energy levels, memory, and mood. The pathway provides tailored rehabilitation support to help you better understand and manage these symptoms, reduce their impact on daily life, and regain confidence in doing the things that matter to you.
Vocational rehabilitation pathway
The vocational rehabilitation pathway is a new, early-intervention programme designed to help working-age people in West Yorkshire stay in or return to work when facing health challenges. Fully funded by NHS England through the Health and Work Accelerator, it offers patient-led, virtual access to support such as assessments, work-focused rehabilitation, and advice on workplace adjustments, aiming to prevent avoidable job loss and reduce pressure on GP services. This integrated approach means patients can access the right rehabilitation support more easily, without needing multiple referrals.
What happens once you’re accepted onto the service
Once you’re part of the service, we’re committed to making your experience as supportive and empowering as possible. Rehabilitation is a shared journey, and like any good partnership, it works best when everyone plays their part. Here’s what you can expect from us, and what we ask from you in return.
What you can expect from us
Kindness, dignity and respect – always.
Personalised support tailored to your health, wellbeing, and life goals.
Care is delivered primarily through small, friendly online groups, where shared experience and peer learning play a key role.
Flexibility and reasonable adjustments to meet your individual needs.
Evidence-based tools and techniques to help you understand your symptoms, build confidence, and take back control of your everyday life.
Our team of clinicians and therapy assistants will guide, support and empower you. We’re not here to replace your GP or medical team, but to help you manage how your condition affects your wellbeing and your ability to live well, and work well.
What we ask from you
Be an active participant. This partnership only works if you’re engaged.
Use your microphone and camera in group sessions when you can – the more present you are, the more you’ll benefit.
Share what you’re comfortable with and respect others’ confidentiality.
Stay open to trying new approaches. Even small steps can lead to meaningful change.
This is a safe and supportive space to build your confidence and improve your daily life. Everyone deserves to feel heard, respected, and supported, especially while navigating long-term health challenges.
Moving forward together
Taking part in the multi-system rehabilitation service means stepping into a supportive partnership where your voice, experience, and effort truly matter. If your goal is to better understand your symptoms, improve daily functioning, or rebuild your working life, you won’t be doing it alone.
We’re here to walk alongside you, offering guidance, encouragement, and tools that work in the real world. Your journey may not be linear, but every small step counts.
We look forward to supporting you as you take control of your health and future.
