Community mental health rehabilitation
In Bradford District and Craven, we are looking to change the way people access mental health rehabilitation
We have recruited two full time rehabilitation specialists, who will be part of the newly created Community Rehabilitation Team and will help deliver the Heathfield Grove project.
Background
In June 2023, Getting It Right First Time (GIRFT) National Mental Health Rehabilitation Forum reported that nationally 79% of NHS trusts had developed community rehabilitation teams. Through the community mental health transformation programme, Bradford District Care NHS Foundation Trust is working alongside colleagues from Bradford Council, the voluntary and community sector, and the NHS West Yorkshire Integrated Care Board to review and strengthen our work on mental health rehabilitation, so that it aligns with Getting It Right First Time recommendations and national guidance.
Rehabilitation and reablement are ways of saying we’re going to help someone get back their skills or the independence they had before. They may have lost their skills and independence due to illness or a difficult experience, and the main goal is to help them get back to doing things for themselves as much as possible.
The care the person receives will be person-centred (providing care that fits their needs), they will be at the heart of all decision making and no decision will be made without their involvement and what they want to achieve.
Our goal is to gradually give people more space and reduce how much they depend on the service, which will help them become more independent. Saying this, we will step in to provide support when needed to lower risks or help them before their condition gets worse. People with more complex needs may need a thorough check of their abilities, like thinking skills or how they process information, before they can start working on their recovery.
What do the Community Mental Health Rehabilitation Team hope to offer?
- We want to provide personalised care that focuses on people getting better, and we want to do this in their communities, closer to where they live
- We will offer thorough assessments and treatments, that will involve different professionals working together to support the individuals needs
- Improve system delivery of rehabilitation focussed treatment alongside keyworkers, influencing and enabling our wider workforce within secondary mental health, the local authority and the voluntary and community sector
- We want to make sure everyone has a fair chance to be healthy, no matter where they live or how much they earn
What is the Heathfield Grove project and who is it for?
- Fourteen supported living tenancies at Heathfield Grove *supported living is a type of housing where people need extra help to live independently (due to an illness or a difficult experience)
- Heathfield Grove refers to the office site where the Enablement and Recovery Team are based. The team are a service provided by the voluntary and community sector and consist of peer recovery workers who provide the day-to-day rehabilitation to the individuals that reside within the flats
- The service is aimed for individuals who meet the Nice Guidelines 181 – Rehabilitation for adults with complex psychosis
- Through the use of collaborative care planning (individuals with mental health conditions work with healthcare professionals and their support network i.e. family to develop and manage their care plan), the individual, the Community Rehabilitation Team and the Enablement and Recovery Team support individuals to holistically address their ongoing needs
- The aim is to support people to regain levels of independence to reintegrate back into the community; whilst also recognising that this may not be to the same level someone once had.
Community Mental Health Rehabilitation: future ambitions
To deliver a high-quality community mental health rehabilitation service, which will meet the needs of the local population. It also means that all healthcare professionals (e.g. occupational therapists, social workers, nurses, speech and language therapists, psychologists, psychiatrists, GPs and pharmacy) involved in the person’s care, will work together to support the individual in their recovery journey.
