Community mental health transformation governance

Our governance is overseeing the changes planned with members for all the organisations involved and has involvement partners supporting the key decisions made.

Our sound governance structure ensures there is accountability for delivery and to provide assurance of patient safety and quality of care.

Our partnership leadership executive (PLE) provides formal leadership for the Bradford District and Craven Health and Care Partnership. Members of PLE lead the local place-based partnership providing a greater focus on population health management, bringing together the work of our health and care providers for the best outcomes for our citizens with a focus on reducing health inequalities. Our core approach is to ‘Act as One’ – putting the needs of our population first and thinking about the whole person. This then drives our work to deliver on our vision of keeping people ‘happy, healthy at home’.

Our PLE reports into our Bradford District and Craven Health and Care Partnership Board. Members of our PLE meet on a monthly basis, the meeting is held in private to allow for careful consideration of plans and proposals as well as ensuring the safe management of risks so that we can safely provide services to our communities. Any decisions that are made at the Healthy Minds Board are taken to PLE

The board understands the needs, sets strategy and resource allocation, accountable for delivery and manage system risk.

This group provides system oversight in their guidance and governance for delivery and funding. The group reviews performance and data, it also identifies opportunities and joint working across Bradford District and Craven Health and Care Partnership, West Yorkshire Health and Care Partnership as well as nationally ie NHS England. It oversees and steer recommendations and escalation from the community mental health transformation delivery groups.

The group provides leadership, accountability, and development for the delivery of workstreams, milestones, and risk logs. It also oversees equality, inequalities and quality impact measures as well as making recommendations for prioritising and planning work.

To develop and collaborate work, co-produce, new ways of working, pathways, and opportunities.

The forum is made up of voluntary and community sector organisations as well as statutory bodies to actively engage and collaborate as a whole system to improve mental health and wellbeing of people living and working across Bradford District and Craven.

Voluntary and community sector organisations will play a pivotal role in the development and delivery of community mental health transformation. We will use the forum as way of sharing information as well as areas of concern.
