Core20PLUS5: Our hyperlocal approach

Our approach to Core20PLUS5 is to reduce health inequalities for those living in the 20% most deprived parts of Bradford District and Craven; to support those in the nationally defined “plus groups” ; and to work towards the 5 NHS England clinical priorities.

To do this we work closely with our thirteen Community Partnerships, aligning the Core20PLUS5 priorities to locally determined plans to reduce inequalities. Resource has been distributed among the Community Partnerships proportionately according to need using deprivation indices.
